Thank you for your willingness to participate in Utah's School Climate Survey. 
 
In House Bill 120: Student and School Safety Assessment Bill (HB120), the State Board of Education was directed to develop an anonymous School Climate Survey to be administered to students, parents, teachers, staff, administrators, and school resource officers. Since this survey will be administered every other year, it is critical to provide feedback. 
 
Please take a few minutes to answer the following questions. The information you provide in this survey will remain anonymous. Your complete honesty will help the school be a safe and welcoming place to be. 
 
The survey varies in a number of questions based on your specific role. It should take 15 minutes or less to complete the survey.
